Front of Entry Form 1. Determine if your project was completed by a SINGLE student or was a TEAM effort. All Lectures are entered as a single entries ONLY! a. Single. If your entry is a SINGLE student project, fill out the STUDENT #1 section. b. Team. If your entry is a TEAM project, the team members should determine who is STUDENT #1, and who is STUDENT #2 and fill in the information for each student. 2. Current Grade Level This is the grade level you are in as of September 1, 2011 3. Current Age Level This is the age you are as of September 1, 2011 4. Home Phone Number This will be used only if we need to speak with a student about the project or lecture demonstration and will be kept private. 5. Citizen of the United States This information is required for the Navy/Marine Award. 6. Title of Entry Please remember that you are limited to 60 characters. The includes spaces, punctuation, symbols, etc. Check to make sure the spelling is correct. 7. Division Please ( X ) the square with the correct Division. 8. Presentation Please ( X ) the square with the correct presentation type. Novices are always Projects. There are two types of presentations for Lectures: School-based (teacher/school sponsored) and Non-school based (organization/scientist sponsored). 9. Subject Please ( X ) the square with the category that BEST fits your Project or Lecture Demonstration.
Entry Form - second page 1. Sponsor Section. A sponsor is the adult who is supervising the student during the scientific process. It may be a classroom teacher, parent/guardian, scientist/researcher, or organization (i.e. Boy/Girl Scouts, 4-H, etc). The sponsor should complete this section. a. Please enter your FULL NAME. We need this for your Certificate and Program Book. Please include: Mr. Mrs. Dr. etc. b. E-mail addresses will help us to keep you informed of any changes. All information in this section is kept confidential. c. Please give the school/organization name, address, and zip code. Supply the sponsor's phone number, starting with area code, in case there are last minute questions or concerns before Congress Day. 2. Check [v/] the boxes answering specific questions about this student's project or lecture. 3. Release Waiver. Unsigned forms will not be registered and the form will be sent back for a signature. If you are a team, both team members may enter on one form or on two individual forms. Each team member's signature is required as well as the signature of each student's parent or guardian and the sponsor. If two forms are used, staple them together. 4. Send the entry with the entry fee to the address located on the bottom of the entry form's second page.
